Many invaluable gains in health related areas have been attained through diligent negotiations. Seeking improved health plan benefits for members of District Council 37 and their families is an ongoing process of top priority.
You must complete a Health and Security Plan Enrollment Card to be eligible for benefits. Be sure to notify the Plan if the size of your family has changed (i.e. marriage, birth, adoption, death, divorce or separation). You do so by filling out a change of status form and providing the proper papers (birth,
marriage, death certificates, etc ).
Proof of schooling is required each semester, to maintain benefits for dependents between age 19 and 23. Remember to notify the Plan of any change of address.

The Booklet contains specific eligibility criteria, benefit limitations and exceptions.
Health and Pension Counseling, Second Surgical Consultation and Social Service Crisis Intervention services (PSU) are also available to eligible employees and their covered dependents.
